4. I am aware that this contribution is not tax deductible.

5. This contribution is made using my personal credit card or debit card for which I have the legal obligation to pay, and is not made using the credit card or debit card of a corporation, labor organization, or another person or entity.

6. This is a campaign contribution for a municipal race for Selectman. I understand that individual donors may not contribute more than $1000 in a calendar year.
